BACKGROUND: Obesity is related to underestimation of one's own weight. AIM: To determine the agreement between nutritional status and self perception of weight status in apparently healthy adults from Talca. SUBJECTS AND METHODS: Data were obtained from a survey applied at 1007 randomly selected subjects, aged 18-74 years old (66% women). The survey identified age, gender, educational level and socioeconomic status. Self-perception of weight was estimated with pictures representing the four nutritional status categories: underweight, normal, overweight and obese. Body Mass index (BMI, kg/m(2)) was calculated to determine nutritional status. Statistical analysis included kappa index, Bowman s symmetry test and a logistic regression model to calculate the odds ratio in different variables related to underestimation of weight. RESULTS: Seventy three percent of subjects were overweight or obese and 44% underestimated their nutritional status. Sixty percent of subjects with a BMI between 25 and 26 kg/m(2), perceived themselves as normal. A higher underestimation was observed in obese subjects (odds ratio (OR) 17.2 95% confidence intervals (CI) 10.9-27.1), in overweight individuals (OR 3.5 95% CI 2.3-5.3), in men (OR 1.5 95% CI 1.1-2.1) and in people aged from 45 to 59 years (OR 1.7 95% CI 1.0-2.7). CONCLUSION: Almost half of studied population had an erroneous self-perception of weight, mainly due to underestimation. Obese subjects, males and people older than 45 years were those with the higher degree of underestimation.

BACKGROUND: Cardiovascular disease (CVD) has several traditional risk factors (RF), and some of them are potentially modifiable. AIM: To determine the prevalence of most common risk factors in adult population in Talca in Central Chile. SUBJECTS AND METHODS: We studied 1007 subjects aged 18 to 74 years (66% women), living in Talca, selected by a probability sampling. They answered a questionnaire and anthropometry, blood pressure, lipid profile and blood glucose were measured. RESULTS: Thirty seven percent of subjects smoked, 70.1% had a body mass index over 25 kg/m(2) and 41% had an abnormally high circumference. High blood pressure was found in 37% (36% of these subjects were unaware of this condition), 44.5% had hypercholesterolemia, 21.5% had low HDL cholesterol, 40.1% had hypertriglyceridemia and 26.3% had high blood glucose levels. CONCLUSIONS: There is a high prevalence of risk factors for CVD in adult population from Talca. The prevalence of risk factors is higher than that reported by the 2003 National Health Survery.

Background: The prevalence of diabetes mellitus in Latin America is not well known. Aim: To study the real prevalence of diabetes mellitus (DM) and its associated risk factors in the VII region of Chile. Material and Methods: A probabilistic sample of 1,325 subjects over the age of 20, stratified by age and differentiated by place of residence was studied. The criteria of the World Panel of Experts convened by the World Health Organization in 1997, was used to define the presence of diabetes (two fasting blood glucose values over 126 mg/dl or a blood glucose over 200 mg/dl, 2 hours after a 75 g carbohydrate oral load). Results: The global prevalence estimated for DM in this population was 5.39 percent. The calculated prevalence in subjects between 20 and 44 years was 1.88 percent (CI 0.39-3.37); between 45 and 64 years, 10.75 percent (CI 8.35-13.14); 65 years or older 11.30 percent (CI 8.00-14.60), p <0.05. The prevalence was 5.8 percent in urban areas and 4.5 percent in rural areas. Forty five percent of diabetics were not aware of their condition. Forty eight percent of diabetics and 31 percent of non diabetics were hypertensive (p <0.01), whereas 47.8 percent of diabetics and 24.6 percent of non diabetics had at least one diabetic parent (p <0.01). Smoking was less frequent in diabetics than in non diabetics (15.7 and 24.3 percent respectively, p <0.05). Ninety percent of both diabetics and non diabetics were sedentary. Discussion: Health care systems require an epidemiological monitoring system to provide information about the prevalence of diabetes mellitus and to control the evolution of patients

RESUMO

Diabetic patients received acarbose, 150 mg/day durign four weeks and this dose was increased to 300 mg/day durign 3 months. Afterwards, patients were followed for a period of 12 weeks without acarbose. Fasting and post-prandial blood glucose and glycosilated hemoglobin were measured sequentially durign the study. Results: Eighty five patients were recruited for the study but 64 complied with the treatment protocol. The age of these patients was 56 ñ 8.8 years old, their diabetes duration was 7.8 ñ 8.8 years and their body mass index was 27.6 ñ 3.6 kg/m². During acarbose treatment, glycosilated hemoglobin decreased from 8.36 ñ 1.33 to 7.71 + 1.7 percent (p < 0.001), fasting blood glucose decreased from 173 ñ 48 to 159 ñ 59 mg/dl (p < 0.03) and post-prandial blood glucose decreased from 254 ñ 80 to 241 ñ mg/dl (NS). After discontinuing acarbose glycosilated hemoglobin and blood glucose levels returned to basal levels. Body weight and blood pressure did not change during the treatment period. Fifty nine patients bad gastrointestinal symptoms (meteorism, flatulence and abdominal distention) that were mild in 59 percent and moderate in 39 percent. Episodes of hypoglycemia were not observed. Conclusions: Acarbose, associated to sylphonylureas is an effective drug to reduce blood glucose and glycosilated hemoglobin levels in patients with non insulin dependent diabetes

RESUMO

We report a 30 years old woman with sporadic poliglandular autoimmune syndrome type II, first seen with an insulin dependent diabetes mellitus and a Grave-Basedow disease that became spontaneously with positive antimicrosomal antibodies. Six years later she presented with persistent vomiting and remarkable reduction in insulin requirements. She had low basal and stimulated cortisol levels and the diagnosis of severe adrenal failure was reached. A CT scan showed normal adrenal glands, she dis not have cutaneous hyperpigmentation nor evidences of mineralcorticoid deficit. A selective autoimmune damage of the fascicular zone was assumed but a selective damage of ACTH producing pituitary cells cannot be discarded. The importance of investigating adrenal function in cases of unexplained reduction of insulin requirements is emphasized
